Clinical effect of different urinary catheter fixation methods in male patients
男性患者不同尿管固定方式的临床效果研究

摘要

目的 通过采取不同的尿管固定方式，对比患者留置尿管期间尿道口渗血、 漏尿、膀胱痉挛的情况，以及不同固定方式下患者不同体位的疼痛分值，为留置尿管患者选择最优的尿管固定方式，提高患者留置尿管期间的舒适度。方法 将泌尿外科手术后留置尿管的180例男性患者纳入研究，随机分为三组，其中A组为对照组，不进行尿管外固定，B组患者尿管固定于腹部，C组患者管固定于大腿内侧，在患者术后第一天进行问卷调查，比较三组患者尿道口渗血，尿道口漏尿，膀胱痉挛发生频率及不同体位下尿道口疼痛情况，研究尿管固定方式对患者舒适度的影响。结果 尿管固定于腹部患者尿道口渗血及膀胱痉挛发生率相较于其他两种固定方式无统计学差异（p=0.563），然而在漏尿发生情况中，固定于腹部的患者出现的比例明显小于固定于大腿及无固定患者的比例（p=0.009），同时，三种不同固定方式下，患者卧位、坐位、立位的NRS疼痛评分中，尿管固定于腹部的疼痛发生率最小。结论 在本次研究中，针对泌尿外科术后留置尿管的患者，尿管固定于腹部的舒适度明显优于固定于大腿内侧及无外固定患者。

Abstract

Objective: By adopting different urinary catheter fixation methods, to compare the urethral orifice oozing, urine leakage and bladder spasm during the indwelling catheter, as well as the pain scores of the patients in different positions under different fixation methods. The patient chooses the optimal urinary catheter fixation method to improve the comfort of the patient during the indwelling catheter.
Methods: A total of 180 male patients with indwelling urinary catheters after urological surgery were included in the study and randomly divided into three groups. Group A was the control group without external fixation of the urinary catheter, patients in group B were fixed with urinary catheters in the abdomen, and patients in group C were The tube was fixed on the inner thigh, and a questionnaire survey was conducted on the first day after the operation to compare the three groups of patients with urethral orifice oozing blood, urethral orifice leakage, bladder spasm frequency and urethral orifice pain in different positions. Results: Compared with the other two fixation methods, the incidence of urethral orifice oozing and bladder spasm in patients with urinary catheter fixed in the abdomen was not statistically different (p=0.563). At the same time, in the NRS pain score of patients in the supine, sitting and standing positions under three different immobilization methods, the incidence of pain with the urinary catheter fixed in the abdomen minimum.
Conclusion  : In this study, for patients with indwelling urinary catheters after urological surgery, the comfort of urinary catheter fixation in the abdomen is significantly better than that in patients with inner thigh and no external fixation.
