Influence of narrative nursing on mental state and quality of life of elderly patients with chronic diseases
叙事护理对老年慢性病患者心理状态及生活质量的影响

摘要

目的 研究分析对老年慢性病患者实施叙事护理的效果以及对其心理状态和生活质量的影响。方法于我院中随机选取64例老年慢性病患者参与实验，收集、分析其基本资料，确认是否符合入组条件。待确认所有患者均符合实验参与条件后，将采用“随机数字法”对其分组，最终分配为两个小组，一组为使用常规护理的对照组，另一组为使用叙事护理的观察组，每组32例，重点评估其心理状态和生活质量改善情况。结果 首先，使用SAS、SDS评分分别于护理前、护理后评估了两组患者的情绪状态，在护理后观察组SAS评分为（45.14±1.77）分，SDS（46.23±2.11）分，明显低于对照组（48.63±2.17）分、（49.63±2.37）分，（P＜0.05）。其次，通过SF-36量表对两组患者精神健康、社会功能、躯体功能、生理功能、生理职能等几个方面的生活质量评分进行了评估比较，其中观察组更高于对照组（P＜0.05）。结论 通过叙事护理的应用有助于减轻老年慢性病患者不良情绪，更有利于提高生活质量水平。

Abstract

Objective To study and analyze the effect of narrative nursing on elderly patients with chronic diseases and its influence on their mental state and quality of life.
Methods: 64 elderly patients with chronic diseases were randomly selected in our hospital to participate in the experiment, and their basic data were collected and analyzed to confirm whether they met the eligibility for inclusion. After confirming that all patients were eligible to participate in the study, they would be divided into two groups using a "random number method" and eventually assigned to two groups: a control group using conventional care and an observation group using narrative care, with 32 cases in each group, focusing on assessing their psychological status and improvement in quality of life.
Results: Firstly, the emotional state of patients in the two groups was evaluated by SAS and SDS before and after nursing, respectively. After nursing, SAS scores of observation group were (45.14±1.77) points and SDS of 46.23±2.11 points, which were significantly lower than those of control group (48.63±2.17) points and (49.63±2.37) points. (P < 0.05). Secondly, SF-36 scale was used to evaluate and compare the quality of life scores of mental health, social function, physical function, physiological function, physiological function and other aspects of the two groups of patients, and the observation group was higher than the control group (P < 0.05).
Conclusion  : The application of narrative nursing is helpful to reduce the ill mood of elderly patients with chronic diseases, and is more conducive to improving the quality of life.
