社区护理服务对社区慢性病管理的意义

摘要

目的 分析社区慢性病管理中社区护理服务的应用价值及效果。方法 选取2022年10月~2023年10月内社区管辖的慢性患者70例，使用随机数字表法分为对照组（常规社区慢性病管理）和观察组（加用社区护理服务）各35例，对比两组管理效果。结果 两组干预前患者心理状态和生活质量评分差异无统计学意义（P＞0.05），干预后观察组患者SAS焦虑评分、SDS抑郁评分低于对照组，QOL生活质量评分与社区护理整体意义评分均显著高于对照组。且观察组患者自我管理能力显著高于对照组组，差异均有统计学意义（P＜0.05）。结论 对社区慢性病患者管理中实施社区护理服务能够显著改善患者心理状态和生活质量，提高社区护理整体意义和患者自我管理能力，具有应用价值。

Abstract

Objective To analyze the application value and effectiveness of community nursing services in the management of chronic diseases in the community. Method 70 chronic patients under community jurisdiction from October 2022 to October 2023 were selected and randomly divided into a control group (conventional community chronic disease management) and an observation group (additional community nursing services), with 35 patients in each group, using a random number table method. The management effects of the two groups were compared. Result There was no statistically significant difference in the psychological status and quality of life scores between the two groups of patients before intervention (P>0.05). After intervention, the SAS anxiety score and SDS depression score of the observation group were lower than those of the control group, while the QOL quality of life score and overall community nursing significance score were significantly higher than those of the control group. Moreover, the self-management ability of the observation group patients was significantly higher than that of the control group, and the differences were statistically significant (P<0.05).
Conclusion   Implementing community nursing services in the management of chronic disease patients in the community can significantly improve patients' psychological state and quality of life, enhance the overall significance of community nursing and patients' self-management ability, and has practical value.
