The impact of humanistic care combined with hospice care on the psychological state and self-esteem level of patients with advanced cancer
人文关怀结合安宁疗护对晚期肿瘤病人心理状态及自尊水平的影响

摘要

探讨人文关怀结合安宁疗护对晚期肿瘤病人心理及自尊的影响，为临床照护优化提供依据。选取2024年1月至2025年1月本院300例晚期肿瘤病人，用随机数字表法分为各150例的对照组和实验组。对照组实施常规护理，实验组在常规护理基础上实施人文关怀结合安宁疗护，干预期为3个月。用症状自评量表（SCL-90）评估心理状态、自尊量表（SES）评估自尊水平，记录两组癌性疼痛数字评分（NRS）及护理满意度，用SPSS 27.0软件分析。干预前，两组SCL-90各维度评分、SES评分、NRS评分无统计学差异（P＞0.05）；干预后，实验组SCL-90核心维度评分显著低于对照组（P＜0.05），SES评分显著高于对照组，NRS评分显著低于对照组，差异均有统计学意义（P＜0.05），且实验组护理满意度显著高于对照组（P＜0.05）。结论：人文关怀结合安宁疗护可改善晚期肿瘤病人心理状态、提升自尊水平、减轻疼痛、提高满意度，值得临床推广。

Abstract

This study explores the impact of humanistic care combined with palliative care on the psychological state and self-esteem of patients with advanced tumors, providing a basis for optimizing clinical care. A total of 300 patients with advanced tumors admitted to our hospital from January 2024 to January 2025 were selected and randomly divided into a control group (n=150) and an experimental group (n=150) using a random number table. The control group received conventional care, while the experimental group received humanistic care combined with palliative care for 3 months. Psychological status was assessed using the Symptom Checklist-90 (SCL-90), and self-esteem was evaluated using the Self-Esteem Scale (SES). The numerical rating scale (NRS) for cancer pain and nursing satisfaction were recorded and analyzed using SPSS 27.0 software. Before the intervention, there were no statistically significant differences in the SCL-90 subscale scores, SES scores, or NRS scores between the two groups (P>0.05). After the intervention, the core subscale scores of the SCL-90 in the experimental group were significantly lower than those in the control group (P<0.05), the SES scores were significantly higher, and the NRS scores were significantly lower, all of which were statistically significant (P<0.05). Additionally, the nursing satisfaction in the experimental group was significantly higher than that in the control group (P<0.05).
Conclusion  : Humanistic care combined with palliative care can improve the psychological state, enhance self-esteem, alleviate pain, and increase satisfaction in patients with advanced tumors, making it worthy of clinical promotion.
