Construction of evidence-based standard operating procedure for IABP maintenance and research on its effect on complication prevention
基于循证的IABP维护标准化操作流程（SOP）构建及对并发症预防效果的研究

摘要

目的 建立基于循证医学的主动脉内球囊反搏（IABP）维护标准化操作流程（SOP），探讨其在并发症防控及治疗安全性提升中的应用价值。方法 选取本院心血管重症监护室收治的72例IABP治疗患者（2024年2月至2025年11月），按随机数字表法分为对照组与观察组，各36例。对照组实施常规IABP维护方案，观察组应用循证构建的IABP维护SOP实施干预。比较两组IABP运行故障发生情况、并发症发生率，及干预前后血流动力学与凝血功能指标变化。结果 观察组IABP运行故障发生率4.44%、并发症总发生率5.56%，均显著低于对照组的22.22%、27.78%（P＜0.05）。干预72h后，观察组平均动脉压（76.34±5.21）mmHg、心排血量（4.89±0.63）L/min，均优于对照组，凝血功能指标波动更平缓（P＜0.05）。结论 循证导向的IABP维护SOP可有效降低设备运行故障及并发症风险，优化患者血流动力学状态，提升IABP治疗的安全性与临床效能，具备临床推广价值。

Abstract

Objective To establish an evidence-based standard operating procedure (SOP) for IABP maintenance and explore its application value in complication prevention and improvement of treatment safety.
Methods 72 patients treated with IABP in the cardiovascular intensive care unit of our hospital (from February 2024 to November 2025) were selected and divided into the control group and the observation group according to the random number table method, with 36 cases in each group. The control group received the conventional IABP maintenance plan, while the observation group implemented the intervention based on the evidence-based IABP maintenance SOP. The incidence of IABP operation failures, the total incidence of complications, and the changes in hemodynamic and coagulation function indicators before and after the intervention were compared between the two groups.
Results The incidence of IABP operation failures in the observation group was 4.44% and the total incidence of complications was 5.56%, which were significantly lower than 22.22% and 27.78% in the control group (P < 0.05). After 72 hours of intervention, the mean arterial pressure (76.34 ± 5.21) mmHg and cardiac output (4.89 ± 0.63) L/min in the observation group were better than those in the control group, and the fluctuation of coagulation function indicators was more stable (P < 0.05).
Conclusion   The evidence-based IABP maintenance SOP can effectively reduce equipment operation failures and complication risks, optimize the hemodynamic state of patients, improve the safety and clinical efficacy of IABP treatment, and has clinical promotion value.
