The impact of hierarchical training management model on operating room safety management and reporting of adverse events
分层培训管理模式对手术室安全管理及不良事件上报情况的影响

摘要

目的 探讨分层培训管理模式对手术室护理人员安全管理能力及不良事件上报情况的影响，为优化手术室培训体系提供科学依据。方法 选取本院手术室2024年1月-2024年12月在岗的80名护理人员作为研究对象，按培训实施时间分为对照组（2024年1-6月，40名），和观察组（2024年7-12月，40名）。对照组采用传统统一培训模式，观察组实施分层培训管理模式，依据职称、工作年限、岗位胜任力划分为基础层、进阶层、精英层，制定差异化培训内容与考核标准。比较两组手术室安全管理质量评分、不良事件上报率及上报及时率。结果 观察组操作规范、风险防控、应急处置、流程优化评分及总分均显著高于对照组，差异有统计学意义（P＜0.05）；组间对比结果显示，观察组不良事件上报率、上报及时率均明显优于对照组，上述指标的组间差异均有统计学意义（P＜0.05）。结论 分层递进式培训管理体系的构建与落地，可显著优化手术室安全管控效能，提升不良事件主动上报的意愿与时效，具备较高的临床转化推广价值。

Abstract

Objective To explore the impact of the hierarchical training management model on the safety management ability and adverse event reporting situation of surgical room nurses, and to provide scientific basis for optimizing the training system of the surgical room.
Methods 80 nurses working in the surgical room of our hospital from January 2024 to December 2024 were selected as the research subjects. They were divided into the control group (2024 January - June, 40 people) and the observation group (2024 July - December, 40 people) according to the implementation time of the training. The control group adopted the traditional unified training model, while the observation group implemented the hierarchical training management model. Based on professional title, working years, and job competency, they were divided into the basic layer, intermediate layer, and elite layer, and differentiated training contents and assessment standards were formulated. The surgical room safety management quality scores, adverse event reporting rate, and timely reporting rate of the two groups were compared.
Results The scores of operation norms, risk prevention and control, emergency response, process optimization, and total score of the observation group were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). The comparison between the groups showed that the adverse event reporting rate and timely reporting rate of the observation group were significantly better than those of the control group, and the differences in these indicators between the groups were statistically significant (P < 0.05).
Conclusion   The construction and implementation of the hierarchical progressive training management system can significantly optimize the safety control efficiency of the surgical room, enhance the willingness and timeliness of active reporting of adverse events, and have high clinical transformation and promotion value.
