Application effect of family participatory transitional care in neonatal intensive care
家庭参与式过渡期护理在新生儿重症监护照护中的应用效果

摘要

目的 分析以家庭为中心的过渡期护理模式在新生儿重症监护室（NICU）出院患儿中的应用效果。方法 选取我院2023年6月至2025年6月期间符合标准的NICU出院患儿及其家庭共72例随机分为干预组与对照组。干预组接受家庭参与式过渡期护理方案，对照组接受常规出院指导与随访。结果 干预组家庭出院准备度量表得分显著高于对照组，出院后主要照护者的育儿压力指数简表得分及非计划性再入院率更低（P<0.05）。结论 家庭参与式过渡期护理能有效提升NICU出院家庭的照护准备度，缓解主要照护者的长期压力。

Abstract

Objective To analyze the application effect of a family centered transitional care model in discharged neonatal intensive care unit (NICU) patients.
Methods A total of 72 discharged NICU patients and their families who met the criteria from June 2023 to June 2025 were randomly divided into an intervention group and a control group. The intervention group received a family participatory transitional care plan, while the control group received routine discharge guidance and follow-up.
Results The intervention group had significantly higher scores on the Family Discharge Preparation Scale than the control group, and lower scores on the Short Form of Parenting Stress Index and unplanned readmission rate for primary caregivers after discharge (P<0.05).
Conclusion   Family participatory transitional care can effectively improve the care readiness of families discharged from NICU and alleviate the long-term pressure on primary caregivers.
