断脐时间对新生儿脐血pH值和血气分析结果的作用分析

摘要

目的 探讨断脐时间对新生儿脐血pH值和血气分析结果的作用分析。方法 于2024年3月-2025年3月期间在本院选择100例新生儿作为研究对象，从中采集脐动脉标本。按孕周分为早产儿50例和足月儿50例，再结合出生1min Apgar评分（8-10分、4-7分）进行亚组分层：即早产儿Apgar8-10分（A组，25例）、早产Apgar4-7扥（B组，25例）、足月Apgar8-10分（C组，25例）、足月Apgar4-7分（D组，25例），两组为早产新生儿，另两组为足月新生儿，分别在胎儿娩出后未断脐立即采集标本（＜10s）、未断脐45s采集标本、脐动脉搏动停止断脐时（60s-90s）采集标本，采集后立即送检，并记录各组脐血pH值。结果 四组在延迟30s时的pH、PaO2、PaCO2、BE值并无显著差异（P＞0.05），但在60s时，A组的数据改善趋势更佳，表示适当延迟断脐时间更有助于促进新生儿的血气平衡（P＜0.05）；A组在1分钟和5分钟的Apgar评分分别高于其他三组，差异具有统计学意义，表示适当的延迟断脐时间可以显著改善新生儿的生理状态，从而提升其早期的适应能力（P＜0.05）。结论 断脐时间对脐血pH酸碱度和血气分析结果有着重要的影响，延迟断脐能够有效促进胎盘输血，以此增加新生儿的血容量。

Abstract

Objective To explore the effect of umbilical cord cutting time on the pH value and blood gas analysis results of neonatal umbilical cord blood.
Methods From March 2024 to March 2025, 100 newborns were selected as research subjects in our hospital, and umbilical artery specimens were collected from them. Divided into 50 premature infants and 50 full-term infants according to gestational weeks, and further divided into subgroups based on the 1-minute Apgar score (8-10 points, 4-7 points): premature infants with Apgar scores of 8-10 points (Group A, 25 cases), premature infants with Apgar scores of 4-7 points (Group B, 25 cases), full-term infants with Apgar scores of 8-10 points (Group C, 25 cases), and full-term infants with Apgar scores of 4-7 points (Group D, 25 cases). The two groups were premature newborns, and the other two groups were full-term newborns. Samples were collected immediately after the fetus was delivered (<10 seconds), 45 seconds before the umbilical cord was severed, and when the umbilical artery pulsation stopped and the umbilical cord was severed (60s). 90s) Collect specimens, immediately send them for testing, and record the pH values of each group's umbilical blood.
Results There was no significant difference in pH, PaO2, PaCO2, and BE values among the four groups at a delay of 30 seconds (P>0.05). However, at 60 seconds, the data improvement trend in group A was better, indicating that appropriately delaying umbilical cord cutting time is more helpful in promoting blood gas balance in newborns (P<0.05); The Apgar scores of Group A at 1 minute and 5 minutes were higher than the other three groups, and the difference was statistically significant. This indicates that delaying the umbilical cord cutting time appropriately can significantly improve the physiological status of newborns, thereby enhancing their early adaptability (P<0.05).
Conclusion   Umbilical cord cutting time has a significant impact on the pH and blood gas analysis results of umbilical cord blood. Delaying umbilical cord cutting can effectively promote placental transfusion, thereby increasing the blood volume of newborns.
