ICU连续性肾脏替代治疗中集束化护理的应用

摘要

目的 分析集束化护理用于ICU连续性肾脏替代治疗期间所起到的作用。方法 随机均分2023年3月-2025年2月本院ICU接诊连续性肾脏替代治疗病人(n=62)。试验组采取集束化护理,对照组行常规护理。对比单个滤器使用时间等指标。结果 关于单次滤器使用时间:试验组长达(22.03±1.74)h,对照组只有(19.02±1.25)h,差异显著(P<0.05)。关于血肌酐和血尿素氮:干预后:试验组只有(135.28±15.06)umol/L、(16.04±3.19)mmol/L,对照组高达(170.47±17.39)umol/L、(24.02±6.41)mmol/L,差异显著(P<0.05)。并发症:试验组6.45%,对照组32.26%,P<0.05。总有效率:试验组96.77%,对照组77.42%,P<0.05。非计划性下机率:试验组0.0%,对照组16.13%,P<0.05。结论 ICU病人连续性肾脏替代治疗期间用集束化护理,单个滤器使用时间更短,并发症也更少,护理效果更好,肾功能改善更加显著,非计划性下机率更低。

Abstract

Objective To analyze the role of bundled care in continuous renal replacement therapy (CRRT) for ICU patients.
Methods A total of 62 patients receiving CRRT at our hospital's ICU were randomly divided into two groups from March 2023 to February 2025. The experimental group received bundled care, while the control group received routine care. Indicators such as the duration of single filter use were compared.
Results Regarding the duration of single filter use: the experimental group had a mean of (22.03 ± 1.74) h, compared to (19.02 ± 1.25) h for the control group, with significant differences (P <0.05). For serum creatinine and blood urea nitrogen: post-intervention, the experimental group had mean values of (135.28 ± 15.06) umol/L and (16.04 ± 3.19) mmol/L, while the control group had mean values of (170.47 ± 17.39) umol/L and (24.02 ± 6.41) mmol/L, with significant differences (P <0.05). Complications: The experimental group had a complication rate of 6.45%, compared to 32.26% for the control group, with P <0.05. Overall effectiveness: The experimental group had an overall effectiveness rate of 96.77%, compared to 77.42% for the control group, with P <0.05. Unplanned extubation rate: The experimental group had a rate of 0.0%, compared to 16.13% for the control group, with P <0.05.
Conclusion   During CRRT for ICU patients, using bundled care results in shorter single filter usage time, fewer complications, better nursing outcomes, more significant improvements in renal function, and a lower unplanned extubation rate.
