延续性护理对出院血液病患者自我管理能力及生活质量的影响

摘要

目的 分析延续性护理对出院血液病患者自我管理能力及生活质量的影响。方法 回顾分析2024年1月-2025年12月院内80例出院血液病患者病历资料，其中2024年1月-2024年12月40例出院血液病患者为常规护理，作为对照组；2025年1月-2025年12月40例出院血液病患者为延续性护理，作为观察组。对比两组患者自我管理能力及生活质量。结果 （1）自我管理能力：观察组护理后自我照护能力量表（ESCA）评分高于对照组（P<0.05）。（2）生活质量：观察组护理后健康调查简表（SF-36）评分高于对照组（P<0.05）。结论 延续性护理对出院血液病患者自我管理能力及生活质量的影响较明显，能够提高自我管理能力，提高生活质量，值得采纳。

Abstract

Objective To analyze the impact of continuity of care on the self-management ability and quality of life of discharged patients with hematological diseases.
Methods A retrospective analysis was conducted on the medical records of 80 discharged patients with hematological diseases from January 2024 to December 2025. Among them, 40 discharged patients with hematological diseases from January 2024 to December 2024 received routine nursing care as the control group; From January 2025 to December 2025, 40 discharged patients with hematological diseases received continuous care as the observation group. Compare the self-management ability and quality of life between two groups of patients.
Results (1) Self management ability: The Exercise of Self-Care Agency Scale (ESCA) score of the observation group after nursing was higher than that of the control group (P<0.05). (2) Quality of life: The SF-36 score of the observation group's post nursing health survey was higher than that of the control group (P<0.05).
Conclusion   Continuity of care has a significant impact on the self-management ability and quality of life of discharged patients with hematological diseases. It can improve self-management ability and quality of life, and is worth adopting.
