摘要
肿瘤患者治疗间歇期居家康复是临床护理的重要延伸环节，而居家期间用药不规范、症状管理缺失、心理压力大等问题常影响康复效果。健康教育作为居家延续护理的核心内容，通过多元模式向患者及照护者传递疾病知识与护理技能，在提升患者自我管理能力、改善生活质量、减少不良事件等方面发挥关键作用。本文结合基层临床实践，从肿瘤患者居家护理现状、延续性护理概念作用、健康教育应用模式、效果分析、现存问题与展望等方面进行综述，为优化基层肿瘤居家延续护理方案提供参考。

Abstract
The period of cancer patients’ treatment break for home rehabilitation is an important extension of clinical nursing. However, problems such as non-standard medication use, lack of symptom management, and high psychological stress during the home period often affect the rehabilitation effect. Health education, as the core content of home continuation care, conveys disease knowledge and nursing skills to patients and caregivers through various modes, playing a key role in enhancing patients’ self-management ability, improving quality of life, and reducing adverse events. This article, based on grassroots clinical practice, reviews the current situation of home care for cancer patients, the role of the concept of continuity care, the application mode of health education, effect analysis, existing problems, and prospects, providing a reference for optimizing the home continuation care plan for cancer patients at the grassroots level.
