Study on the current status and influencing factors of preoperative patient activation among young and middle-aged patients undergoing gastroscopy and colonoscopy
中青年胃肠镜检查术前患者积极度现况及影响因素研究

摘要

目的 探讨中青年胃肠镜检查术前患者的积极度现状并分析其影响因素，为制订针对性的护理干预策略提供参考依据。方法 采用便利抽样法，选取2025年3-8月在宁夏某三甲医院消化内镜中心接受胃肠镜检查的198例中青年患者作为研究对象。采用一般资料调查表、患者积极度量表及医院焦虑抑郁量表进行调查并应用多重线性回归分析法得出患者积极度的影响因素。结果 中青年胃肠镜检查术前患者的积极度得分为（64.52±13.89）分，其中27.78%的患者积极度处于低水平。多重线性回归分析结果显示，文化程度（β=0.223，P=0.001）、焦虑评分（β=-0.289，P<0.001）、抑郁评分（β=-0.212，P=0.002）及是否参加健康教育培训（β=0.115，P=0.044）是中青年胃肠镜检查术前患者积极度的独立影响因素。结论 中青年胃肠镜检查术前患者的积极度处于中等水平，受文化程度、焦虑抑郁情绪以及健康素养等多种因素影响。在临床工作中，护理人员应根据该群体的个体特征，制订有针对性的护理干预策略，通过健康教育、心理疏导等措施，提高患者的术前积极度，确保检查顺利进行。

Abstract

Objective To explore the current status of preoperative patient activation in young and middle-aged patients undergoing gastroscopy and colonoscopy, and analyze its influencing factors, so as to provide evidence for formulating targeted nursing intervention strategies.
Methods A total of 198 young and middle-aged patients who received gastroscopy and colonoscopy in the Digestive Endoscopy Center of a tertiary hospital in Ningxia from March to August 2025 were enrolled by convenience sampling method. The general information questionnaire, Patient Activation Measure and Hospital Anxiety and Depression Scale were adopted for investigation. Multiple linear regression analysis was used to identify the influencing factors of patient activation.
Results The preoperative total score of patient activation among young and middle-aged patients was (64.52±13.89), and 27.78% of the patients had a low level of patient activation. Multiple linear regression analysis revealed that educational level (β = 0.223, P = 0.001), anxiety score (β = −0.289, P < 0.001), depression score (β = −0.212, P = 0.002), and participation in health education and training (β = 0.115, P = 0.044) were independent influencing factors of preoperative patient activation among young and middle-aged patients undergoing gastrointestinal endoscopy.
Conclusion   The preoperative patient activation of young and middle-aged patients undergoing gastrointestinal endoscopy is at a moderate level, which is affected by education level, anxiety and depression, health literacy and other factors. In clinical practice, nurses should formulate individualized and targeted nursing intervention strategies based on the characteristics of this population. Measures such as standardized health education and professional psychological counseling should be implemented to improve patients’ preoperative activation and ensure the smooth completion of examination.
