The impact of community chronic disease management on glycemic metabolism indicators and lipid levels of prediabetic patients
社区慢性病管理对糖尿病前期患者糖代谢指标、血脂水平的影响

摘要

目的 探究在糖尿病前期患者的治疗中，社区慢性病管理措施的实施对患者糖代谢指标和血脂水平的影响。方法 筛选2025年2月至2026年2月社区筛查中收治的60例糖尿病前期患者纳入样本展开研究，使用随机数字表法进行分组，30例/组，对照组给予常规健康宣教，观察组在对照组基础上采取社区慢性病管理措施，比较两组患者干预前后的糖代谢及血脂水平差异。结果 干预前，两组患者的糖代谢指标及血脂水平间均无显著差异（P>0.05）；干预后，两组患者的各项糖代谢指标和血脂水平均得到显著改善，但观察组空腹血糖（FPG）、餐后2小时血糖（2hPG）、糖化血红蛋白（HbA1c）指标及总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）指标更低，且高密度脂蛋白胆固醇（HDL-C）指标更高（P<0.05）。结论 在糖尿病前期患者的治疗中，社区慢性病管理措施的实施可有效降低糖代谢指标，改善血脂水平，值得推广应用。

Abstract

Objective To explore the impact of community chronic disease management measures on glucose metabolism indicators and blood lipid levels in patients with prediabetes during treatment.
Methods Sixty patients with prediabetes admitted during community screening from February 2025 to February 2026 were selected as the study sample. They were grouped using a random number table method, with 30 patients in each group. The control group received routine health education, while the observation group underwent community chronic disease management measures in addition to the control group’s treatment. The differences in glucose metabolism and blood lipid levels between the two groups before and after intervention were compared.
Results Before intervention, there were no significant differences in glucose metabolism indicators and blood lipid levels between the two groups (P > 0.05). After intervention, both groups showed significant improvements in various glucose metabolism indicators and blood lipid levels. However, the observation group had lower Fasting Plasma Glucose (FPG), 2‑hour Postprandial Plasma Glucose (2hPG), Glycated Hemoglobin A1c (HbA1c), Total Cholesterol (TC), Triglyceride (TG), and Low‑Density Lipoprotein‑Cholesterol (LDL-C) levels, and higher High‑Density Lipoprotein‑Cholesterol (HDL-C) levels (P < 0.05).
Conclusion   In the treatment of patients with prediabetes, the implementation of community chronic disease management measures can effectively reduce glucose metabolism indicators and improve blood lipid levels, making it worthy of promotion and application.
